Monarch Physical Therapy and Wellness is an out-of-network provider. This means that we provide a superbill for physical therapy services, but do not interact directly with your insurance provider.

This allows Monarch Physical Therapy and Wellness to be upfront about costs (no surprise hefty bills down the road), provide comprehensive care (evaluate and treat you or your child as a whole movement system vs one body part), and deliver care at the frequency and duration that you need (no more 6 visits to address 20+ years of chronic pain and dysfunction).

Monarch Physical Therapy and Wellness is a mobile practice. We offer easy, convenient treatments in your home, office, gym, or wherever you are having your pain/mobility concerns. Mobile visits are offered within a 30-minute radius of downtown Grand Rapids. Private treatment space can be arranged for those living outside of the travel radius upon request. Monarch Physical Therapy and Wellness also offers virtual treatment to anyone living in the state of Michigan.

No! You are able to access physical therapy directly in the state of Michigan. Some restrictions apply when continuing care beyond 21 days. Please reach out if you have any questions.
